CO2激光治疗睑缘色素痣的临床观察

摘要
目的: 观察超脉冲CO2激光治疗睑缘色素痣的疗效。方法: 麻醉成功后用功率1~2 W的超脉冲CO2激光非接触式照射病灶直至将肉眼可见的色素组织完全清除。灰线处病灶的处理原则为保证色素完全去除的前提下尽量使病变处灰线和病灶两端正常的灰线相融合呼应。如色素累及睑结膜则先处理灰线外侧病灶再处理睑结膜色素。术后给予氯霉素滴眼液滴眼直至伤口愈合。结果: 64例患者78个病灶完成随访; 优: 32个(41%); 良: 28个(35.9%); 中: 12个(15.4%); 差: 6个(7.7%)。结论: CO2激光治疗睑缘色素痣, 治疗精准, 可在去除靶组织的同时塑造眼睑形态, 手术操作简单, 损伤小, 伤口愈合快, 感染发生率低, 术后眼睑形态良好, 美容效果佳, 是一种值得推荐的治疗方法。
Abstract
Objective: to observe the effection of superpulsed CO2laser in treating palpebral margin melanocytic naevus. Methods: all lesions were local anesthesia before the treatment. The 1~2 W power superpulsed CO2 laser was applied to ablate the melanocytic tissues in non-contact mode until all pigment tissues disappeared. The treatment principle is to make the margin smooth on the premise of completely removing all the pigment tissues. If the pigment tissues involved in the palpebral conjunctiva, the outer lesions were first treated and then the palpebral conjunctival target tissues. After that, the target eyes were given Chloramphenicol Eye Drops three times every day until the wound healed. Results: of the 64 patients with 78 lesions, 32(60.9%)of them got excellent results, 28(35.9%)of them got good results, 12(15.4%)of them got moderate results and(7.7%)of them got poor results. Conclusions: it is an accurate and simple way in treating palpebral margin melanocytic nevus with superpulsed CO2 laser. It can help to shape the eyelids into their original form with the advantage of minimize the side effects and attain a desirable cosmetic outcome.
