Medical femtosecond laser devices are used in dermatology for non-linear high-resolution imaging to obtain non-invasive and label-free optical skin biopsies (multiphoton tomography) as well as in ophthalmology for refractive corneal surgery and cataract surgery. Applications of commercial certified multiphoton tomographs include early detection of skin cancer within minutes by two-photon autofluorescence imaging of coenzymes and melanin and second harmonic imaging of collagen as well as by testing the efficacy of pharmaceutical and cosmetical products. Goals are (i) to reduce the number of physically taken human skin biopsies in hospitals and research institutions, (ii) to optimize personalized medicine, and (iii) to reduce animal studies in pharmacy. Current diagnostic tools in dermatology include surface microscopy with a dermatoscope and ultrasound but have poor resolution. Optical coherence tomography and confocal reflectance microscopy have better resolution but provide limited information based on changes of the intratissue refractive index. Multiphoton tomography provides the best resolution of all clinical imaging methods and offer functional imaging such as optical metabolic imaging based on autofluorescence lifetime imaging. Goals of femtosecond laser eye treatment are (i) the replacement of mechanical microkeratomes for corneal flap generation, (ii) the replacement of the UV nanosecond excimer laser for stroma removal, and (iii) to replace, in part, the scalpel in the surgery of cataracts and other eye diseases. So far, millions of eye treatments have been conducted around the world. The major disadvantage of current certified medical femtosecond laser devices is the high price compared with the standard mechanical and optical medical devices.

研究了幼儿先天性白内障术后无晶状体眼的角膜散光与屈光散光的关系。通过分析术后无晶状体眼幼儿的检影验光与角膜曲率的测量结果,使用Thibos向量分析使之成为J0和J45分向量,再对角膜散光和整眼屈光散光的向量组进行回归分析,发现纳入研究的55只眼(30个幼儿)的整眼屈光散光和角膜散光的结果(平均值±标准差)分别为(-1.35±0.73) D和(-1.96±0.92) D,相应的轴位为(125.18°±63.97°)和(120.78°±62.75°),整眼屈光散光向量组(RJ0,RJ45)与角膜散光向量组(CJ0,CJ45)有显著的回归关系。因而,可以通过Thibos向量分析法,用角膜散光来预测整眼的屈光散光。

目的: 报告1例飞秒激光治疗移植物抗宿主病相关性白内障患者。方法: 对一名男性异基因造血干细胞移植术后2年患者的白内障情况进行诊治并追踪。患者, 男, 29 岁, 异基因造血干细胞移植术后2年, 双眼视力渐进性下降1年余, 影响日常自理生活。眼部体征: Vod:0.06→0.2, Vos:CF/眼前(矫不应); NCT:R 11.2 mmHg, L 16.7 mmHg, 泪液分泌试验: R 5 mm/5 min, L 2 mm/5 min, 双眼结膜混合充血(++++), 右眼晶体后囊下皮质呈砂锅底样混浊; 左眼角膜中央实质层云雾状混浊并见大量新生血管, 晶体全白色混浊。诊断: 1.双眼并发性白内障, 2.左眼角膜斑翳, 3.慢性移植物抗宿主病, 4.急性淋巴细胞性白血病, 5.异基因造血干细胞移植术后, 6.糖尿病。予右眼行白内障超声乳化摘除联合人工晶体植入术, 左眼行飞秒激光辅助白内障超声乳化摘除联合人工晶体植入术。结果: 患者术后视力Vod: 0.4, Vos: 0.04, 目前患者生活自理已完全不受影响。结论: 飞秒激光辅助白内障超声乳化摘除联合人工晶体植入术对特殊患者亦具有很高的安全性及手术效果。

随着人口老龄化,年龄相关性白内障患者日益增多,通过植入人工晶状体可以使白内障患者的视力基本得到恢复。由于白内障患者对术后的视觉质量要求越来越高,个性化设计的人工晶状体已逐渐在临床上推广使用,而多焦点人工晶状体的出现,则成功解决了单焦点人工晶状体术后难以解决的近视力问题。对人工晶状体的发展历史做了简要的阐述,详细地说明了单焦点和多焦点人工晶状体的设计思想及特点,介绍了多焦点人工晶状体的发展现状,最后对人工晶状体未来的发展进行了展望。

目的: 对飞秒激光辅助超声乳化手术相较与传统的扭动超声乳化白内障手术的优越性进行综述。方法: 对国内外发表过的关于飞秒激光辅助下白内障超声乳化手术相关的多篇研究进展及论文进行综合性整理及分析。结果: 飞秒激光完成白内障手术中切口制作、前囊膜切开、晶状体切削、加或不加角膜缘松解等步骤, 减少了人工手术中的不确定性因素, 使白内障手术更加精准, 安全性更高, 使术中及术后并发症均减少。结论: 飞秒激光辅助下的白内障手术较传统超声乳化白内障手术有明显的优越性, 即将成为当今白内障手术的主流。

Dynamic light scattering (DLS) is a promising technique for early cataract detection and for studying cataractogenesis. A novel probabilistic analysis tool, the sparse Bayesian learning (SBL) algorithm, is described for reconstructing the most-probable size distribution of α-crystallin and their aggregates in an ocular lens from the DLS data. The performance of the algorithm is evaluated by analyzing simulated correlation data from known distributions and DLS data from the ocular lenses of a fetal calf, a Rhesus monkey, and a man, so as to establish the required efficiency of the SBL algorithm for clinical studies.

研究一种针对白内障患者的潜在视功能检测仪。采用透射式视力分辨率板并且使进入眼瞳成像光束足够细和有足够的光能量,以致能够通过白内障患者云雾状晶状体中的微小间隙在视网膜上成像,移动透射式视力分辨率板调焦可测出患者的视度在-7D~+12D之间的潜在视力。进行了光学系统结构设计和理论计算,进行了实验和测试。实验测试结果表明,该系统出瞳位置光斑直径=0.094 mm,测得视力与传统测试方法测得视力相一致。通过该仪器对白内障患者手术前检测,可比较准确预测置换人工晶体手术后达到的视力水平,这对医生和患者都具有重要参考价值。
