激光诱导击穿光谱技术在生物医学中的研究进展
1 引言
生物体内元素组成以及含量变化与生物代谢、疾病发展的相关性被广泛研究,同时也成为医学诊断和治疗的重要支撑信息[1]。研究发现,一些必要的生物元素被证明参与氧化还原反应[2]、调节生物代谢平衡[3]、控制生物过程[4],并具有适应性免疫调节功能[5]。例如,使用动物模型和细胞培养模型的临床研究表明,铜作为各种酶促反应和生理活动的重要辅助因子,具有神经毒性,诱导炎症因子释放并产生反应性自由基,这可能促进肿瘤形成[6],并增加阿尔茨海默病的风险[7]。而锌被证明了在过敏、传染病治疗和疾病预防等方面具有积极效果[8]。此外,这些生物元素浓度的失衡可导致机体紊乱,并最终导致一些危及生命的疾病,如炎症、动脉硬化、心肌梗塞,甚至癌症[9]。因此,生物元素在生理和病理代谢过程中至关重要。
生物元素的测定有多种化学分析方法,如原子荧光光谱法(AFS)、电感耦合等离子体-光学发射光谱法(ICP-OES)、X射线荧光光谱法(XRF)、电感耦合等离子体质谱(ICP-MS),原子吸收光谱法(AAS)和气相色谱-质谱法(GC-MS)等[10-13]。尽管这些分析技术中的大多数具有较高的敏感性和精确性,但是这些方法耗时较长、样品制作复杂、需要化学实验才能给出定量结果,并且检测设备昂贵,这限制了它们的应用范围[10]。因此,研究人员不断寻求新的分析和诊断技术,提高生物元素测定效率,以便能够在实验室以及临床环境中快速、准确地测量。
激光诱导击穿光谱(LIBS)作为一种原子光谱分析技术,能够实现快速、原位、多元素同步的痕量检测,并能以微损的方式为几乎任何样本提供定性和定量信息,且不需要制备大量的样本。LIBS因其操作简单,适用于固体、液体、气体和气溶胶[11-13],已广泛应用于冶金工业、环境保护、生物医学、考古学、航空航天、农业和食品安全等领域[14-16]。特别地,LIBS技术的独特优势使其在生物医学方面有非常好的应用前景。本文就LIBS的技术特点介绍其在生物医学中的血液检测、生物组织分析、元素成像中的应用,并基于研究现状对LIBS技术在生物医学应用上的发展趋势进行分析与展望。
2 LIBS技术概述
2.1 LIBS技术的基本原理
LIBS是一种利用激光实现光谱化学分析的技术,将足够能量的激光通过聚焦透镜聚焦于样品表面,使样品被烧蚀并产生发光等离子体,当激光脉冲终止时,等离子体开始冷却。在等离子体冷却过程中,激发电子状态下的原子和离子的电子落入自然基态,导致等离子体发射具有离散光谱峰的光[17]。等离子体发射的光被采集模块收集并传送到光谱仪,然后这些光被分离成不同的波长并入射到电荷耦合器件探测器上转换为电子信号。计算机软件可以利用这些信号绘制光谱图,显示原子、离子和分子片段发射的光线的强度和波长。该特征光谱的波长对应于元素的类型,特征光谱的相对强度对应于元素的浓度[10]。利用这些光谱信息可以快速识别和定量分析样品中存在的元素及其化学组成。LIBS技术原理如
2.2 LIBS技术在生物医学中的分析方法
LIBS通过检测样本元素的光谱数据实现元素的定性和定量分析。在定性分析中,使用LIBS检测到的谱线鉴别所代表的元素,并与数据库中的数据进行对比以进一步分类。一般而言,在分析光谱之前会对光谱进行校正、过滤和异常值去除等。为了提高分类的准确性,研究人员开始逐渐使用机器学习算法来分析光谱信息以确定物质的种类。例如主成分分析(PCA)、独立成分分析(ICA)、K均值聚类、二次判别分析(QDA)、最近邻分类算法(KNN)、偏最小二乘判别分析(PLS-DA)、支持向量机(SVM)、最小二乘支持向量机(LS-SVM)、分层聚类分析(HCA)等[15]。
目前,LIBS在定性分析方面有无可比拟的优势。然而,等离子体演化过程中不可避免的形态波动严重影响了LIBS的定量性能[19]。因此,为了提高LIBS的定量性能,LIBS的定量算法被广泛研究。其中,免校准分析方法(CF-LIBS)可能是多年来在LIBS领域中最受关注的定量方法之一[20]。CF-LIBS方法具有实时、原位、现场、单点、多元素分析等诸多优势,在生物医学领域具有巨大的应用潜力[21]。近年来为了提高定量分析的准确性和可重复性,人工智能算法已成为LIBS定量分析的最新热点,如偏最小二乘回归(PLSR)、人工神经网络(ANN)、逻辑回归(LR)、多元线性回归(MLR)、随机森林(RF)等[22]。

3 LIBS技术在血液检测中的研究进展
检测和诊断血源性疾病对患者治疗和确保国家供体血液供应安全至关重要。血液作为微量元素的运输媒介能够反映血液系统性疾病,提供机体代谢时元素浓度变化的直接证据[37],全血、血浆和血清都是检测机体痕量元素的可靠样本。在过去的几年中,已有许多研究者评估了血液中微量金属的价值[1,38]。尽管目前有许多血液元素检测方法,但临床诊疗仍然需要更低成本、更快速、更简单的分析方法。
3.1 基于LIBS技术的血液肿瘤检测
恶性肿瘤的早期诊断对于有效治疗和控制癌症的进展非常重要,血液检测能够辅助实现癌症的早期诊断,而LIBS技术可以利用血液中元素的变化实现肿瘤诊断。Chen等[39]使用LIBS技术结合PCA、线性判别(LDA)和KNN模型,准确识别了全血样本中的淋巴瘤,分类准确率超过99.7%,并发现淋巴瘤肿瘤组的Fe I、Na I和K I谱线弱于对照组。他们还使用KNN模型在9 min内从16 μL血清样本中识别出淋巴瘤和多发性骨髓瘤(MM),总体准确率为96.0%[40],这在临床上具有重要的应用价值。为了研究LIBS技术在诊断MM血清分期上的作用,在另一项研究中,该团队使用LIBS结合KNN、SVM和ANN算法对光谱数据进行分类,MM分期的准确率超过90%[30]。在针对LIBS技术对多种血液肿瘤的检测能力方面,Chu等[41]开发了一种随机子空间结合线性判别分析(RSM-LDA)的模型用于鉴别血清样本中的急性髓性白血病(AML)、慢性粒细胞白血病(CML)以及MM和淋巴瘤,平均准确率达到了98.34%,如

3.2 LIBS技术在血液检测中的其他应用
LIBS技术在血液疾病检测中还有着更广泛的应用。随着中国人口老龄化的加剧,阿尔茨海默病(AD)也逐渐成为中国人第5大死因,早期诊断和对AD与其他痴呆的明确区分至关重要。Gaudiuso等[43]利用LIBS诊断AD,以Na、Mg、Ca、CN等元素的差分光谱作为输入数据,结合二次判别分析(QDA)进行分类,准确率达到80%。Berlo等[44]将LIBS与PCA-LDA模型相结合区分新冠病毒阳性和阴性供体的血浆,准确率高达95%,并发现SARS-CoV-2阳性受试者样本中的Zn和Ba含量减少,且与LIBS中的CN谱线呈负相关。在药代动力学中,LIBS也具有广阔的应用前景,Chu等[45]利用LIBS量化血液中的药物半衰期,使用SVR进行LIBS定量分析,得到MnO2-BSA的半衰期为2.49 h,AlO(OH)- BSA半衰期为3.46 h。与ICP-MS相比,LIBS的相对误差在5%以内,如

LIBS技术结合机器学习方法,能够在几分钟或几小时内快速识别血源性疾病并获得分析结果,可以大大提高识别、控制和治疗疾病的能力,有望成为恶性肿瘤血液快速、准确诊断的强大工具。现阶段,化学计量学模型多种多样,针对不同样本特点选择不同的化学计量方法对LIBS分类结果的提高至关重要。如何稳定提升LIBS技术的血液快速检测能力,无疑是化学计量学在LIBS数据分析中的发展方向之一。此外,LIBS技术在血液元素检测方面的应用需要结合实际需求,同时还需要不断探索新的研究方向。
4 LIBS技术在生物组织中的研究进展
近年来,生物元素作为细胞和生物组织重要组成受到越来越多研究者的关注。与传统组织病理学分析相比,LIBS多元素分析为疾病诊断提供了一种新颖的方法。现阶段,LIBS技术组织检测主要聚焦在软组织和硬组织两个方面,软组织以病理诊断和生理检测为主,硬组织主要对钙化组织进行研究[47-48]。本小节探讨LIBS技术在软组织上的病理诊断、病理划分和其他生物组织病理检测的应用现状,探索其在提高疾病诊断准确性方面的潜力。
4.1 LIBS技术软组织检测
4.1.1 病理诊断
LIBS技术结合机器学习可作为肿瘤组织诊断的潜在工具。研究发现,甲状腺癌(PTC)组织在LIBS光谱中的Ca和Mg水平显著高于正常组织,通过SVM分类器和LDA分类器区分PTC组织和正常组织,总精度分别达到92.6%和81.5%[49]。在宫颈癌研究中,Wang等[50]利用LIBS验证了人体宫颈癌组织中Na、Mg、K的归一化峰强度显著高于正常组织,而正常组织中Ca的归一化峰强度高于宫颈癌组织,使用PCA-SVM和SVM鉴别的准确率分别为94.44%和93.06%。在黑色素瘤的鉴别中,Khan等[51]将LIBS数据和拉曼光谱数据融合,利用元素的原子和分子结构信息实现黑色素瘤和正常组织的辨别和分类,使用ELM分类模型实现了拉曼光谱和LIBS的组合数据的最高准确度(100%)。为了评估肿瘤治疗效果,Lin等[52]通过LIBS分析化疗前后的肿瘤组织元素以预测肿瘤治疗情况。结果表明,化疗前后样本之间的光谱差异位于300~700 nm范围内,且Ca是区分化疗前后组织的最重要元素,这可能与Ca元素在药物治疗时对血管生成的调节有关[53]。LIBS技术结合化学计量法和多光谱技术在癌症组织鉴定方面具有潜力,使LIBS技术为实时诊断癌症组织提供了初步的研究依据。
4.1.2 病理划分
LIBS通过检测肿瘤和边界组织中的微量元素,为肿瘤手术切除提供了有价值的参考。比如,肺肿瘤和边界组织表现出相似的颜色和纹理,特别是在边缘区域,这导致外科医生无法精准识别。针对这一现状,Lin等[54]利用LIBS结合不同的机器学习方法来实现肺肿瘤和边界组织之间的区分。他们采用SVM和Boosting Tree模型,并使用随机森林进行特征选择,准确率达到98.9%。同样,Teng等[55]提出一种基于LIBS的脑胶质瘤浸润边界术中鉴别方法,以辅助检测手术中难以确定的肿瘤浸润边界,减少复发率和死亡率。结果表明,LIBS结合SVM和特征选择方法实现了高的分类率,其神经胶质瘤和边界组织的正确分类率达到95%,如

4.2 LIBS技术用于其他生物组织检测
LIBS检测不仅适用于软组织,还可以扩展到其他生物组织,包括头发、指甲和硬组织,因为它们相比血液或软组织在更长的时间尺度上记录了饮食史和生理状况,并且易于获得的样本使其成为LIBS分析的理想目标。Cherni等[57]首次使用LIBS检测研究了头发对2型糖尿病和骨质疏松症[35]的诊断潜力,确定了Mg和Na的组合可能适合于诊断2型糖尿病,Ca和Na是骨质疏松的有效生物标志物。该方法通过分析头发中的矿物质元素诊断早期疾病,是一种很有前途的新方法。为了精确定量分析头发中的矿物元素量,Zhang等[58]开发了一种有前途的方法,通过超声波辅助碱溶解(UAAD)来增加Zn和Cu的谱线强度,如

LIBS 技术与机器学习方法相结合,可以成为识别生物组织中元素、实现组织划分和疾病诊断的强大工具。但是生物组织样品表面的质地、粗糙度和非均匀性会影响激光能量与组织的耦合以及等离子体的生成过程,导致LIBS光谱曲线波动[55],因此临床条件下的样品制备是LIBS在软组织元素分析的一大挑战。此外,现有设备多用于实验室的稳定环境,不适用于临床检测,急需研发一种可携带、对微弱信号敏感的LIBS在线检测仪器[48]。同时,根据不同的样本特点构建可靠的算法模型并收集足够多的样本数据也至关重要。
5 LIBS技术在元素成像中的研究进展
在生物医学领域,光学成像技术因其优异的灵敏度、高特异性和高分辨率而备受关注[61]。鉴于元素失衡与疾病的潜在关系,LIBS成像技术对生物元素在空间上的异质性分布检测和深入研究将有助于医疗诊断。该技术利用激光诱导的等离子体在样品表面的不同位置生成光谱,并从中提取与感兴趣元素相关的线强度,再构建元素图以获得相应的元素图像[62],LIBS成像仪器和原理如

5.1 LIBS成像在病理划分中的应用
LIBS技术在内源性和外源性多元素原位成像能力已被广泛研究,人们能够通过元素分布的不同区分皮肤癌与健康皮肤组织。研究发现与健康皮肤相比,皮肤癌中的Ca、P、Na和Mg浓度范围波动较大,并且在所有肿瘤区域观察到高水平的P[67],如

LIBS成像技术可以揭示组织内积聚的外源性元素的存在,这对药物和纳米颗粒的研究至关重要。le Guével等[70]在不同时间段对小鼠不同器官进行LIBS多元素成像,发现肾脏、肝脏和脾脏中Au的变化情况不同,揭示了Au纳米颗粒在组织中的清除率和含量。在用于辐射剂量增强的稀土复合纳米闪烁体的治疗价值研究中,Busser等[71]将LaF3∶Ce纳米颗粒注射到大鼠胶质母细胞瘤,然后用同步辐射X射线治疗。LIBS图像显示,在放射治疗前肿瘤区域聚集了大量的纳米颗粒,但还有一些肿瘤细胞仍然没有纳米颗粒或很少的纳米颗粒,这为验证靶向治疗的效果提供了新的检测手段。在研究抗肿瘤治疗过程中,Wei等[72]利用LIBS多元素成像显示不同方法治疗后肿瘤组织样本的元素差异。结果表明,DNA水凝胶/DOX被证明是抗肿瘤治疗的最佳方案,可促进肿瘤中Ca的富集和Mg含量的相对降低。此外,H&E染色和IHC分析表明,肿瘤组织和周围组织之间的病理形态没有显著差异,但LIBS元素图中Mg和Ca的相对丰度不同。该方法可能是评价抗肿瘤治疗效果和机制研究的有效途径。
5.2 LIBS三维成像在生物医学中的应用
LIBS成像技术可以进行深度成像分析,以确定内源性元素和外源性元素的扩散行为及其深度边界,这种方法对于生物组织的深入分析尤其适用。Gimenez等[66]以纳米颗粒(NPs)作为载体,开发了两种三维成像方法,实现了器官尺度上组织中的NPs的成像,如

LIBS元素成像能够以时间和空间两种维度对组织元素分布进行研究,在空间上实现原位肿瘤边缘划分、临床复杂多样的病理标本元素分布成像,在时间上研究纳米颗粒药物在不同时间段组织的分布以评估药物对肿瘤的治疗效果,特别是金属基纳米颗粒的临床药代动力学和毒性研究。因此,基于LIBS技术的影像学检查与常规组织病理学分析的互补性可能有助于快速、准确诊断,符合未来精准医学的发展趋势。此外,LIBS成像技术需要进一步提高其稳定性和准确性。正如Gardette等[74]在系统性回顾LIBS成像技术在生物医学应用中指出:“大量数据的处理是当前该技术发展及其在实验室外应用的障碍,化学计量法的使用,无疑是未来几年的主要研究挑战。”总体而言,LIBS元素成像技术具有很大的应用潜力,可以为病理学、临床研究和医学领域的疾病治疗提供更全面的组织学信息。
6 总结与展望
生物医学领域作为LIBS技术应用的新兴研究领域,已经取得了一些具有实质意义的研究成果。然而相比于已将LIBS投入实际应用的其他领域,LIBS技术在生物医学领域的应用仍然存在许多问题亟待解决。
在血液检测方面,LIBS技术结合机器学习算法可以实现较高的精度,但由于缺乏多中心样本支持和血液样本数量的限制,LIBS技术在稳定性和准确性方面仍面临挑战,在之后的研究中,可以进一步探索多样本(如尿液、汗液和唾液等)联合检测方法以提高检测的可靠性。在组织病理诊断方面,生物组织的柔软性和高度异质性等制约了LIBS在线检测的发展。而机器学习和目标识别算法或将突破这些限制。在医学成像中,LIBS成像需要较长的测量时间,且缺乏组织的形态学信息。之后的研究可以探索多光谱技术的结合,以增强LIBS对元素的识别能力。此外,新的光谱处理方法和数据分析技术也可以进一步提高LIBS成像技术在生物医学研究中的应用价值。
未来,LIBS技术有望成为液体活检和组织活检技术的有益补充,可提供快速、准确的诊断结果,并在医学成像上与现有的影像技术形成互补。化学计量学的发展也为LIBS技术在生物医学研究中的应用提供更多可能性。综上所述,在生物医学领域中,未来LIBS技术应以临床需求为导向,以性能提升为目标,开发专业化特种仪器以在临床应用中展现更广阔的前景,为实现精准医疗提供强有力的支撑。
