光学相干层析显微内窥成像技术研究进展(特邀)创刊六十周年特邀
1 引言
临床医学在体成像技术对疾病的发现、诊断和治疗具有重要意义。医院常用的在体成像技术包括X射线计算机断层成像(X-CT)、核磁共振成像(MRI)、超声成像(US)[1],但以上非光学技术仍然存在测试安全性较差、成像分辨率不足等问题。例如:X-CT需要人体组织暴露在辐照下,增加了病患辐射风险;MRI扫描时间长,无法真正做到实时成像;US分辨率低,只能获取宏观的病变组织信息。生物医学光学成像技术具有非侵入性、无辐射、高分辨率和实时成像的优点,通过检测生物组织对光的吸收、散射、反射、荧光等特性可实现透射式、反射式和荧光成像[2]。透射式成像技术如共聚焦显微成像技术可实现亚微米级的高分辨率成像,但通常只适用于离体样品的病理学分析,无法完成体内原位实时成像,且穿透深度浅。根据有无标记物,荧光分子成像可以分为自发荧光成像和外源荧光成像。外源荧光成像需要特定的标记物,可能引发人体免疫性问题;自发荧光成像对组织具有选择性,信噪比低。且它们都存在穿透深度不足的问题。因此对于光学成像技术,如何在保证高分辨率的同时增加成像深度是非常重要的命题。
光学相干层析成像(OCT)是一种反射式医学成像技术,该技术基于低相干干涉原理,采用近红外宽带光源,通过检测待测样品的后向散射光,重建样品的微观结构图像。OCT具有无损伤和高分辨的断层图像获取能力,无须切取组织也能获得活体组织的微细结构信息,可实现对人类活体组织或器官的“光学活检”,极大减轻了患者的病痛并提高了疾病的发现和诊断效率。OCT的典型分辨率可达10 μm左右,可以在3 mm左右深度上实时测量一维深度方向图像、二维断层图像及三维体积图像。与X-CT、MRI、US等传统非光学在体成像技术相比,OCT的成像深度较小,但其分辨率获得了至少1个数量级的提升[3],如
OCT目前已广泛应用于眼睛、皮肤等易于接触或观察的体表器官和组织的病变检测,该类型OCT系统的成像探头多采用振镜扫描式物镜,体积较大,无法深入到人体内器官进行检测。光纤内窥OCT是基于光纤传输和光纤探头成像的内窥显微成像技术,除了具有OCT的一般成像优点外,还具有体积小、质量轻、耐腐蚀、电绝缘、抗电磁干扰等特点,尤其适用于对现有其他成像技术无法到达的狭小腔道内的组织病变进行高分辨率检测和早期诊断[5]。光纤内窥探头的独特优势是正好可以用于对人体内狭小器官内病变的检测,如检测血管内脂质沉积、斑块、纤维冒等病变。得益于光、机、电、算技术的进步,光纤内窥OCT在21世纪获得蓬勃发展:光纤OCT系统从时域OCT发展到频域OCT,成像速度和成像分辨率都获得显著提高;光纤OCT探头从光纤-棱镜组合探头发展到一体化光纤复合型探头,分辨率和焦深得到进一步提升,探头尺寸得到进一步减小。本文从光纤OCT系统设计及工作原理、光纤内窥探头设计及制备技术、内窥成像应用三方面综述近年来光纤内窥OCT的发展现状,重点总结了光纤内窥探头的发展及其在医学诊断中的应用,并结合前沿报道和应用需求展望了未来光纤内窥OCT的发展方向。
2 光纤OCT系统设计
光纤OCT系统决定了成像速度和纵向分辨率。光纤OCT系统使用单模光纤、光纤耦合器等光纤器件替代传统的分束镜、耦合透镜等体积功能部件,简化了系统。光纤OCT系统本质上是一个低相干干涉仪(LCI)。组织深度方向上的一维信号(A-scan)可以通过干涉门控(时域OCT)和不同的波长(频域OCT)编码得到,通过移动样品或者扫描入射光可以获得组织的二维横截面图(B-scan),通过螺旋扫描可获得组织的三维体图像。光纤OCT系统设计和发展先后经历了时域OCT(TD-OCT)、谱域OCT(SD-OCT)和扫频OCT(SS-OCT),如

2.1 时域OCT
TD-OCT是最早的OCT,于1991年由Fujimoto团队[6]首次报道。TD-OCT多使用具有高斯谱型的宽带光源,在干涉信号分析中,宽带光源可以看成是多个单色光源的叠加。因此TD-OCT中干涉项信号可以表示为

1991年Fujimoto团队[6]首次将TD-OCT用于对人眼视网膜、冠状动脉的离体测量。此后,TD-OCT系统作为商用OCT设备先后应用于眼科和心血管疾病的早期临床研究,包括黄斑[7-8]、青光眼[9]及动脉斑块的影像诊断。TD-OCT系统受限于参考臂反射镜的机械扫描形式,A-scan扫描速率慢,仅能达到数百或数千赫兹[10-11],难以满足大范围三维成像的需求。此外,TD-OCT的探测灵敏度较低(~90 dB),严重阻碍了OCT在体内成像应用的推广。
2.2 频域OCT
FD-OCT的概念同样在1991年被提出,但当时受限于激光器和信号探测器的技术发展,直到1995年后FD-OCT才相继用于人眼视网膜[12]、皮肤[13]结构测量中。FD-OCT系统发展的黄金时期是2003年前后,扫描阵列探测器和扫频光源技术的进步推动了OCT在临床医疗应用方面的全面发展。FD-OCT直接测量干涉信号光谱,样品的深度信息通过对干涉信号光谱进行傅里叶变换得到。FD-OCT干涉项信号可表示为

根据实现方式的不同,FD-OCT可分为SD-OCT和SS-OCT。SD-OCT使用宽谱激光器和扫描阵列探测器作为信号光光源和探测器,利用光谱仪在空间尺度上分光,扫描阵列探测器上的不同像素单元记录不同波长的干涉光谱。SS-OCT使用扫频激光器和单点探测器作为信号光光源和探测器,利用单点探测器在时间尺度上分光,顺序记录不同时刻扫频光谱出射的不同波长光的干涉光谱。它们的基本结构如
对于SS-OCT系统,整体性能依赖于扫频光源质量,包括线宽、频谱带宽、扫频速度和噪声等参数,缺乏可靠的扫频光源一直是SS-OCT的发展瓶颈。早期SS-OCT的A-scan扫描速度不高,不仅成像性能比SD-OCT更差,而且扫频激光器的价格昂贵。直到2005年后随着基于旋转扫描的多面镜滤波器[16]、傅里叶域锁模(FDML)[17]、垂直腔面发射激光(VCSEL)[18]的扫频光源技术的发展,SS-OCT系统的A-scan速率逐渐提升至100~400 kHz,灵敏度达90 dB~110 dB,具有比SD-OCT更优秀的roll-off表现,自此获得了业界更多的关注和临床应用。目前SS-OCT的发展重心依然在光源上,许多扫频光源已经集成了线性波数模块,省去了硬件、软件插值矫正的麻烦,借助现代计算机的强大算力,甚至可以实现百MHz扫描速度[19]。然而,主流扫频光源的中心波长仍限制在1310 nm,扫频带宽限制在100 nm,仅有少部分短腔扫频光源实现了150 nm带宽。相较于可自由选择带宽范围的SD-OCT,SS-OCT纵向分辨率较难提升。对于SD-OCT与SS-OCT系统,拓展光源带宽并增大光源功率是未来的重要发展方向。
3 光纤内窥OCT成像探头研究现状
受限于光纤器件的制备工艺,早期光纤内窥OCT探头在光纤端面集成了扩束透镜、聚焦透镜、反射棱镜和旋转扫描装置等微小光学元件和功能器件,然后整体封装在一个成像导管内。OCT探头根据探测光束出射方向的不同,可分为

3.1 光纤-棱镜组合型探头
早期第一代光纤OCT探头是由光纤和分立的体积光学元件粘连组成的光纤-棱镜组合型探头。1996年Tearney等[20-21]报道了一款用于血管内成像的侧视OCT探头,如

为了深入分析病变发生发展机制,了解疾病演化进程,需要OCT具备细胞甚至亚细胞尺寸的成像分辨率。2014年Xi等[32]在成像探头中加入

光纤-棱镜组合型OCT探头为实现高质量成像奠定了基础,但对棱镜的加工尺寸和精度要求高,光纤与棱镜装配难度大、可靠性低,使用光学胶粘连时会额外引入胶与透镜面的背向散射,降低系统灵敏度。同时棱镜等分离光学元件的引入也限制了光纤探头的微型化和集成度,因此这种类型OCT探头适用于较大尺寸器官的内窥成像。为了解决探头的加工装配问题,进一步减小探头尺寸,提高探头的集成度,提升探头可靠性,第二代全光纤型OCT探头设计逐渐成为了研究热点。
3.2 全光纤型探头
全光纤型OCT内窥探头采用GRIN光纤或光纤透镜作为聚焦元件,可直接通过光纤熔接机连接,极大简化了OCT探头的制备工艺。2002年Reed等[37]首次报道了基于GRIN光纤的全光纤型OCT侧视探头,他们将探头封装在127 μm内径的医用注射器内监测了活体小鼠的脑部运动。GRIN光纤的聚焦能力由光纤芯径和折射率渐变函数共同决定。2007年Mao等[38]详细探讨了扩束光纤(spacer)和GRIN光纤的几何参数变化对成像光斑和工作距离的影响,如

除GRIN光纤外,光纤球透镜同样可以用来聚焦光束。球透镜的焦距可通过控制放电功率和放电次数来调节,通过侧面抛磨制备侧视型球透镜。2010年Mao等[41]首次报道了高成像质量的光纤球透镜探头,实验结果表明该探头具有与GRIN光纤探头相近的成像性能。2011年Yoo等[42]首次将光纤球透镜与近红外荧光成像结合进行冠脉血管内成像,该探头实现了30 μm横向分辨率和4.6 mm成像深度。2017年Yuan等[40]分析了球透镜的圆锥系数以及工作距离与轴向色差的关系,制备了一款消色差的侧视型OCT球透镜探头,如
对于全光纤探头,GRIN光纤因有限的数值孔径难以实现长焦深、高横向分辨率的探头设计。通过增加相位掩模板(PM)或将出射光束整形为贝塞尔光束可有效拓展焦深。2012年Lorenser等[43]在GRIN光纤透镜或spacer后加入相位掩模板,如

全光纤型探头制备工艺简单、成本低、尺寸小、可集成度高,可实现狭小腔道的成像,是当前光纤OCT的研究热点。但球形光纤透镜和GRIN光纤透镜因自身抛物线型折射率分布都不可避免地产生像差,降低成像分辨率。为了提高成像分辨率,增加光源带宽和减小中心波长是最有效的方法,但随着光源带宽的增加、中心波长的减小,色散和像差问题也越发突出。与此同时,伴随成像分辨率的提高,焦深减小,组织深度方向上可清晰成像范围大幅度缩小。因此,如何优化色散、矫正像差、化解成像分辨率和焦深的矛盾是未来光纤OCT研发的重要方向。
3.3 光纤复合型OCT探头
光纤复合型OCT探头是一种利用现代先进光刻技术制备的内窥OCT探头,包括激光光刻、电子束光刻、飞秒激光双光子聚合3D打印等,利用光刻胶、光学薄膜制备的微结构来改善光纤OCT探头的色散与像差问题,化解焦深与成像分辨率的矛盾,进一步提升OCT内窥成像探头的成像性能。2016年Xing等[46]首次采用激光光刻技术在GRIN透镜端面制备二元相位滤波器(BPSF),如

双光子聚合(TPP)3D打印技术是通过飞秒激光诱导光刻胶改性实现微光学元件制备的增材制造工艺。TPP具备超高的加工精度(最小特征尺寸~10 nm)和真三维加工的特征,配合计算机辅助设计(CAD)、扫描振镜、三维运动控制系统,飞秒激光可在透明光刻胶内直写定制的三维结构,满足各种功能化微光学器件的制备需求,实现如微粒操控[49-50]、光束整形[51-53]、微透镜成像[54-55]等功能,扩展了功能性微纳器件在光纤端面集成的应用前景。利用TPP工艺可直接在光纤基底上增材打印各种复杂结构的微透镜,可实现自由曲面镜头的高精度制备与集成,实现色散补偿、像差矫正、焦深拓展等功能。2018年Li团队[56]首次报道了双光子聚合3D打印OCT探头,如

借助超表面、自由曲面或衍射微光学元件的光纤复合型OCT探头,以及具有不同阿贝数材料的复合材料探头可以矫正像差和色差[58-60],并拓展焦深,是未来光纤OCT探头设计和制备技术最有前景的研究方向。
4 光纤OCT在医学影像检测中的应用
光纤OCT可实现对人体内狭小腔道内活体组织或器官的“光学活检”,在临床医学检测中具有广泛的应用前景,如呼吸道系统、消化道系统、泌尿系统和心血管系统等。尤其在心血管系统中的应用,血管内OCT有望替代血管内超声成为心血管疾病检测的金标准。
4.1 呼吸系统
组织病变多始于上皮细胞层,OCT在人体组织的穿透深度一般不超过3 mm,但足以满足大部分上皮细胞检测的需求,已逐渐成为多种疾病常规检查和术后复检评估的重要手段。OCT可原位测量气管等腔体上皮细胞层的增生情况和成熟状态,将上皮细胞的非典型增生与癌变组织从正常组织、增生和变性的组织中区分开来,锁定疑似癌变的病灶区,提高肺癌诊断的准确性[61-62]。

4.2 消化系统
在消化系统方面,OCT在胃肠道研究中常用于检查和诊断食管、小肠、结肠、胆管等管腔组织病变,针对消化道不同位置癌变组织进行识别与诊断,对食管癌、结肠癌等疾病的早期筛查具有重要意义[72]。

在胃肠道检查中OCT的另一潜在应用是对结肠息肉的检测与诊断[74]。目前常规检测方法是在肠镜的辅助下通过穿刺从结肠中进行疑似病灶区取样,然后通过组织病理学检查来确定它们是恶性肿瘤还是良性肿瘤。但一些常见的癌前病变如无蒂锯齿状腺瘤、乳突锯齿状腺瘤只表现出细微的组织学特征,也不易通过普通内窥镜检查识别出来。光纤内窥探头可借助结肠镜深入病灶区,实现对肠内息肉的原位检测,通过深度层析黏膜层结构变化,辅助诊断结肠内的增生性息肉组织与癌变组织。

4.3 泌尿系统
研究人员在对人和猪的输尿管进行研究时发现,内窥OCT可以清晰分辨黏膜层、肌层、外膜等输尿管壁结构,如

4.4 心血管系统
心血管疾病(CVD)是影响人类健康的头号杀手,中国存在心血管疾病危险因素的人群巨大,截止至2019年,我国心血管疾病的患病人数已达2.9亿,其中患冠心病的人数已达1100万[88]。冠状动脉疾病(CAD)是典型的高致死率心血管疾病,其病理基础是冠状动脉粥样硬化引起的血管变窄与堵塞,会造成心肌缺血、缺氧,甚至猝死。心血管疾病检测是光纤内窥OCT最有发展前景的一项应用,血管内OCT(IVOCT)未来有希望成为心血管疾病诊断的“金标准”,相比传统的冠状动脉造影术(CAG)和血管内超声(IVUS)成像技术,其分辨率最高可达几微米,能够轻松分辨在不同病理基础下的细胞行为以及微观结构,诸如纤维组织、钙化组织、脂质核心、胆固醇晶体、巨噬细胞浸润、红色血栓与白色血栓等,非常适用于冠状动脉斑块分析。根据不同结构的OCT图像特征识别动脉粥样硬化斑块种类,为判断冠状动脉病变程度提供重要依据。
2019年Yin等[35]研发了基于FMI的高分辨、长焦深OCT探头,并对人类尸体冠状动脉进行了测试,获得了超高分辨率血管内断层图像,如

IVOCT常用于辅助PCI手术,是目前唯一能精确提供血管支架贴壁以及内膜增生信息的影像学工具。临床研究表明,在血管支架植入、扩张过程中使用IVOCT引导的诊疗效果不亚于传统的IVUS、CAG影像技术的效果[89]。在术前可利用IVOCT高精度地扫描管腔形态与尺寸,判定斑块性质,为植入支架的长度与大小提供依据;术中即时评估支架的膨胀不良、贴壁不良、组织脱垂等问题,及时对手术进程进行干预和调整;在术后对支架的安全性进行长期随访[89-90],检查愈合过程中支架的形态[91],避免支架内血栓[92]的形成。

5 光纤内窥OCT未来发展方向
“早发现、早诊断、早治疗”是疾病诊疗的总策略。疾病的早期诊断要求医学影像检测技术必须做到安全、高分辨率、大成像深度、多维度、快速实时成像。光纤内窥OCT具备微创和高分辨率的断层图像获取能力,无须对组织进行穿刺取样就能获得活体的细微结构信息,将管腔器官疾病的检测方式从繁杂的组织病理学活检程序中解放出来,不仅提高了诊疗效率,还极大减轻了患者的疼痛,在体内疾病诊断、病理研究、新型医疗药物与器械的研发中发挥了重要作用。目前光纤OCT内窥成像技术在对疾病的早期诊断和高精度诊断中仍受以下两点制约。1)分辨率受限,焦深不足,且成像分辨率和焦深相互制约。一方面减小中心波长和增加带宽可以提高OCT轴向分辨率,但同时又会放大色散、像差等成像质量问题,造成OCT探头横向分辨率降低。另一方面增加成像探头数值孔径可以提高横向分辨率,但焦深受到抑制。2)图像信息少。OCT仅能提供散射光强度相关的深度信息,不具备对组织蛋白、脂质等特异性分子的识别能力,难以分析组织的化学组成特征。未来,在先进光刻制备工艺以及多种成像技术的加持下,光纤内窥OCT有望在探头设计和关键成像性能上取得新突破。
5.1 高分辨率和长焦深成像
发展高分辨率和长焦深的可视化成像工具对观察精细病变结构与动态演化、了解组织病变的发展过程、研究包括心血管疾病在内的管腔类疾病的发病机理、医疗器械和药物的干预作用等关键问题具有重要意义。研发高分辨率OCT尤其μOCT需要使用较短波长和较大带宽光源[94],由此引入的色散问题异常突出,系统内始终存在宽带信号与色散管理之间的矛盾,相应光学器件的频谱特性需要精确控制、系统色散需要精确补偿,才能确保μOCT系统达到细胞甚至亚细胞成像分辨率,获得媲美病理学分析分辨率的斑块组织图像。
2016年Yuan等[95]研究了宽带超连续光源(中心波长为800 nm,带宽为~246 nm)的最佳工作条件,实现了高分辨率(轴向~2.7 μm)、高灵敏度(~107 dB)、高速成像(7×104 A-scan/s)的内窥OCT系统。2017年Park等[96]使用消色差双胶合透镜组来补偿系统色散,结合衍射器件的内窥探头实现了更高轴向分辨率(~2.4 μm)的OCT系统。此外光纤探头受限于自身微小尺寸,像差矫正困难,色散和像差问题造成OCT成像分辨率显著降低。成像分辨率和焦深亦相互矛盾,高数值孔径的物镜可以有效提高探头的横向分辨率,但同时会减小成像系统的焦深,必须另辟蹊径保证高分辨率且长焦深的成像性能。随着光纤探头制备以及激光微加工技术的发展,研究人员研发了结合衍射透镜[32]、光纤球透镜[40]、BPSF[46-47,97]等探头结构的光纤μOCT系统,这些系统均在微米级别成像分辨率下实现了焦深延展的成像效果。然而探头设计中仍未完全解决减小光源波长和增大光源带宽引起的色散、像差等成像质量问题,成像分辨率仍有待提高。2019年Tearney团队[35]报道了基于FMI光纤探头的第二代长焦深μOCT系统,如

5.2 多模态集成一体化成像
多功能集成、小型化、一体化是光纤探头未来的重要发展方向。将OCT与其他成像技术复合组成多模态内窥成像技术,通过不同成像技术的优势互补,可以弥补OCT成像穿透深度不足、获取组织信息量少的缺陷,有助于全面评估冠状动脉易损斑块的几何形貌、化学组成、生物力学特性。但如何高效地融合不同工作机理的成像技术并集成于一体式光纤探头中,增强光纤内窥OCT系统的多模态成像性能,仍是一项重大挑战。
IVUS具有比OCT更大的穿透深度,两者在分辨率与成像深度上互补,可实现近场的高分辨率成像与远场的高穿透成像,可精确测量纤维帽厚度来分析易损斑块的几何形貌。2010年Li等[98]报道了一种基于OCT与超声的双模态血管内成像系统,如

荧光分子成像可以增强OCT诊断时的分子特异性,通过检测特定分子的自发荧光信号,提高对蛋白、纤维、脂质等结构化学组成的识别能力。2011年Yoo等[42]报道了一款结合近红外荧光成像与OCT的双模态内窥成像导管,利用近红外荧光成像提供蛋白酶、纤维蛋白等额外的分子敏感信息,帮助识别冠状动脉中的高风险斑块。2022年Li团队[100]在双包层光纤上同时集成了OCT成像镜头与荧光成像镜头,制备了一种可进行OCT与荧光双模态成像的OCT内窥探头,如

6 结论
光纤内窥OCT具备无创或微创的高分辨率断层图像获取能力,同时具有系统构造简单、探头尺寸小、质量轻、电绝缘和抗电磁干扰等优点,非常适用于对现有其他成像技术无法到达的狭小腔道内的组织病变进行高分辨率检测和早期诊断。光纤内窥OCT对疾病诊断、病理研究以及探究人体对新型药物治疗和医疗器械的反应具有重要意义,在呼吸系统、消化系统、泌尿系统,尤其在心血管系统的内窥检测中表现出广阔的应用前景。未来,随着光源、耦合器、探测器等关键光器件的发展,光纤OCT系统的分辨率和成像速度将得到进一步提升。与此同时,将超表面、自由曲面等微光学元件复合到光纤探头中有助于进一步提高成像分辨率,同时延长焦深。此外,光纤OCT与荧光、超声、光声成像等技术融合并制备的高度集成的一体化多模态内窥成像探头可同时提供组织病变的多维度信息,如组织成分的形态学特征、化学组成、生物力学特性等,进一步提高对组织病变的识别能力与诊断效率。综上所述,新一代光纤内窥OCT的发展将进一步拓展临床应用场景,为疾病的病理研究和医生诊疗提供更直观、科学的影像依据,为人民的生命健康安全保驾护航。
